Admissions

The admissions process commences with a referral packet containing pertinent information about your patient. By providing us with these key medical documents, we are better able determine the level of care that your patient requires, as well as utilization of their health benefits. If necessary, an on-site assessment by our clinical liaison may be requested, in order to ensure accuracy. We request that you submit the following documents within your referral:

Transition from Hospital

  • Face Sheet/Demographics Info
  • Most Recent History & Physical (H&P)
  • List of Medications
  • Recent Nursing Notes
  • Recent Labs, if applicable
  • List of Isolations, if applicable
  • Any Advanced Directives on File (i.e. POA, DNR, etc.)
  • Name and Date of agency that completed the Pre-Admission Screening (PAS)

Transition from Rehab/Nursing Facility

The admissions processes commences with a referral packet containing key information about the patient. We request that you submit the following documents within your referral:

  • Face Sheet/Demographics Info
  • Most Recent History & Physical (H&P)
  • List of Medications
  • Recent Nursing Notes
  • Recent Social Service Notes
  • Recent Labs, if applicable
  • List of Isolations, if applicable
  • Any Advanced Directives on File (i.e. POA, DNR, etc.)
  • Copy of OBRA and Pre-Admission Screening (PAS)
  • Verification of Income

Coverage

Glenwood HealthCare and Rehab is certified to participate in both Medicare and Illinois Medicaid programs. We are also contracted to work with many private insurers, the Veterans Administration (VA) and Hospice companies. Please contact a member of our Admissions Team with any questions or concerns you have regarding financial options.

We participate in the following reimbursement plans:

  • Medicare*
  • Medicaid
  • Medicaid Managed Care Organization Plans: Aetna Better Health and Illinicare
  • Veteran's Administration Contracts
  • Blue Cross Blue Shield Health Insurance and many other Private Health Insurance Plans**
  • Hospice
  • Private Pay

*Medicare eligibility requires a three-night consecutive stay in the hospital, after being admitted.
**Please contact our Admissions office to determine eligibility of your insurance plan.
